Service Description:
|
Provides community-based support for young people living with mental illness and substance use challenges, including those transitioning between the youth and adult mental health systems.
The program serves youth aged 15-26 who live in the west end of Toronto (west of Yonge St. to Kipling Ave., south of Eglinton to the Lake) and are living with mental health and substance use issues.

Self-referrals are encouraged. Referrals can also be made by family members, physicians, hospital professionals, mental health and community organizations.
Referrals are made though The Access Point.
