Service Description:

Scarborough Centre for Healthy Communities works in collaboration with numerous partner agencies, referring practitioners to support timely access for patients to inter-professional primary care services.
The IPPC Team is an Ontario Health atHome initiative.
IPPC team consists of:
- system navigators
- physiotherapist
- social workers
- mental health caseworkers
- harm reduction workers
- registered nurses
- registered dieticians
- nurse practitioners
- foot care nurse
- chiropodist
