Service Description: Offers a range of support and services for transitional aged youth (ages 15-24) with mental health and/or substance abuse issues. It is offered in collaboration with the Black Creek Community Health Centre.


Application: Clients looking to access this program must contact The Access Point. Referrals are accepted from family members, friends, self, community agencies, physicians, professionals, lawyers, etc.
