Rehabilitation services provided in the patient's home or other non-clinical locations. Services can include physiotherapy, occupational therapy, speech language pathology, case management, and vocational or other evaluations. Publicly funded in-home rehabilitation services may be arranged by Ontario Health atHome.

These services are located outside of Durham North East, but provide service to Durham North East.
