Service Description: Group Homes - eight homes located in Port Hope and Cobourg for 41 individuals.

Residential Program - Clients along with their residential staff, enjoy planning vocational, recreational and leisure events and opportunities both within their home and other community settings. Such events may include dances, movies, volunteering, community events and sporting events, as well as parades.

Supported Independent Living Program - assists clients in meeting their individual goals by working with social service programs and related organizations to obtain other needed supports. SIL program addresses accommodation, financial, health care, social, employment and case management.

Community Advocacy Program - enables clients to live their lives to the fullest as independently as possible, within their community. APS workers provide support in case management, money management, outreach, accommodation, health care coordination , instrumental counselling, emotional counselling and advocacy.

Fees: There is a fee for some recreational activites. Please call organization for details.
