Service Description: GAIN Teams (located both in hospitals and the community) provide comprehensive geriatric assessments and care planning and delivery to address the complex needs of many older adults. The inter-professional teams work with older people and their families to develop care plans that support frail seniors with complex health issues so they can remain living at home. Staff include: Nurse Practitioners, Care Coordinators, Physiotherapists, Occupational Therapists, Social Workers, Pharmacists, Personal Support Workers, Geriatrician and physician support and Administrative team members.
