LEVELS OF CARE: This facility offers three levels of care:

LIGHT: Person requires minimal assistance or supervision with activities of daily living and availability of 24 hour staff for safety and security.

MEDIUM: Person requires assistance with all activities of daily living with possible exception of skin care or, if cognitively impaired, individuals may require less assistance but supervision and frequent directions. Generally these individuals are easily redirected and exhibit no aggressive behaviours, and only slight-moderate agitation

HEAVY: Person requires considerable assistance with all activities of daily living and may also be cognitively impaired

SPECIAL UNITS: 51 bed unit

BATHROOM TYPES: Private and shared

TOUR FREQUENCY: Upon arrangement

FACILITY SECURITY TYPE: There are two secure home areas with gardens for residents who are cognitively impaired

RESIDENT CARE SERVICES: This facility provides the following care: routine aseptic dressings, removal of staples or sutures, sterile dressings, care and removal of drains, colostomy/ileostomy care, dialysis diet, foot care, prosthesis care, intermittent catheterization, indwelling catheterization, suprapubic catheterization, incontinence supplies, nephrostomy tube, oxygen concentrator, liquid oxygen, inhalation therapy, palliative care, suctioning (oral), tube feeding, tracheostomy care, care of methicillin-resistant Staphylococcus aureus (MRSA) positive residents

FACILITY LIFESTYLE - PERMISSIONS: Smoking is permitted in an out door restricted areaif resident passes a smoking assessment Smoking is not permitted indoors. There is a bar/social hour at the facility

FURNITURE POLICY: Residents may bring their own furniture but some restrictions apply

MEAL PLAN / SERVICE TYPES: Three healthy and delicious meals are provided each day, as well as snacks. Each home area has a central dining room. Meal tray service is available for ill residents. A snack area, family dining, special diets and limitations are available

PHYSICIAN VISIT FREQUENCY: Family physicians are permitted. Name(s) of Facility Physicians: Doctor Millar, Doctor Silverberg, Doctor Liston and Doctor Lunn and Doctor Lukinuk

RESTRAINT METHOD: No vest restraints. Lap belt 2 side rails, charit tray (attached or tied, tilt chair, comfy chair, gerichair (broda) with either a lap belt or attached tray and secure unit
