Service Description: Adult day program * social, recreational, and therapeutic activities for Alzheimer's Disease, dementia, acquired brain injury, the frail elderly, cognitively impaired and adults with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care * hot meal and snacks provided

Fees: $21 per day
Fee Reduction: Notice of Assessment from Revenue Canada is required by the Adult Day Program Coordinator to determine approval for subsidy

Eligibility / Target Population: Older adults and adults with disabilities, who need physical, social, or emotional supports * Ontario Health atHome care coordinators assess the needs of applicants * clients must be medically stable and not requiring nursing care * must be able to function in a group setting * one person transfers only
