Service Description: Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services. They also assist the client in planning for discharge when services are no longer required.

Care coordinators are also responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services. The services provided through Ontario Health atHome may include:
  • personal support
  • nursing
  • physiotherapy
  • occupational therapy
  • speech-language therapy
  • social work
  • dietetic services
  • mental health services
  • case management/coordination
  • medical supplies and dressings
  • hospital sickroom equipment
  • laboratory and diagnostic services
  • determination for drug coverage under the Ontario Drug Benefit (ODB) Plan
  • Determines eligibility for Adult Day Programs

Fees: Covered by OHIP
Application: Contact Central Intake at 1-800-263-3877 to request an assessment for services.
Waiting Period - No waiting period to make a new referral. Waiting periods for direct service vary depending on type of service required and priority need.
Eligibility / Target Population: Visiting services are available to residents of Ontario of any age who have a valid Ontario Health Insurance (OHIP) card. Further eligibility criteria will be assessed by a case manager during the intake process based on the full eligibility criteria as
