Service Description:
|
The Stroke/Rehabilitation units are supported by an interdisciplinary care team that provides care and service to adult patients recovering from:
- Stroke
- Neurological Disorders
- Guillain Barre Syndrome
- Multiple Sclerosis
- Parkinson's Disease
- Peripheral Neuropathy
- Peripheral Neuropathy
- Amputation
- Orthopedic surgery or trauma
- General Disability i.e. post operative strengthening
The care team includes:
- Physiotherapists and assistants
- Occupational Therapists and assistants
- Speech Language Pathologist (Dysphagia)
- Rehab Nurses
- Social Worker
- Ontario Health atHome Care Coordinator
- Physicians
- Clinical Nurse Educator
