Service Description:

GAIN provides comprehensive geriatric/senior assessments for those with multiple, complex health concerns.

GAIN addresses complex health concerns such as:

memory loss or confused thinking
walking and mobility
falls or risk of falls
bladder or bowel control
multiple medications
difficulty coping
safety issues
frequent hospital visits
