Service Description: Outpatient Services

Services offered in clinic setting and in the home when patients are no longer physically able to come to the clinic
  • complex pain and symptom management
  • support with establishing goals of care and advanced care planning
  • support with addressing one’s physical, spiritual, emotional, social and informational needs
  • support for patients and families facing difficult end of life decisions such as withdrawal of treatment
  • procedures such as paracentesis and blood transfusions

"About Us"
Supportive Palliative Care Patients referred for palliative care are those suffering from incurable and/or life-limiting illnesses of a cancerous and/or non-cancerous nature. Palliative Care is appropriate for patients in all disease stages, from those undergoing active medical treatment to those who are nearing the end of life. At all stages, the goal is to provide pain and symptom management thereby improving quality of life
