Service Description: Services for people and families facing life-limiting illness, death and bereavement * in-home hospice palliative care support throughout Scarborough

Two programs with various opportunities:
  • Hospice Palliative Care - one-on-one in-home visits, Day Hospice, Caregiver support group
  • Bereavement Care - one-on-one visits, various support groups

There are four volunteer roles available and extensive training is provided:
  • a hospice home visiting volunteer providing emotional, social, spiritual and practical (non-medical) support for people living with life-limiting illness, their caregivers and families
  • a day hospice volunteer providing support and helping to coordinate activities for people who have been diagnosed with a life-limiting illness and attend a once a week day hospice program
  • a hospice caregiver group facilitator co-facilitating a once a week support group for people who are caring for a loved one who has been diagnosed with a life-limiting illness
  • a hospice bereavement care peer facilitator providing one-on-one peer support and/or co-facilitating mutual support bereavement groups for people who have experienced the death of a partner/spouse, parent, sibling or child of any age

Application: Call for self-referral * service providers can use the referral form on the website.
Eligibility / Target Population: Adults who can read and speak English but also may speak different languages to meet the needs of clients and the needs of the volunteer role
