Service Description:
|
Supports for people living with type 1 and 2 diabetes, gestational diabetes, prediabetes or those at risk of developing diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian, and foot care nurse on staff * services offered virtually or by telephone
- support groups such as Just the Basics, Heart Health, Peer Support
- insulin pump starts and support
- insulin and non-insulin injections
- oral hypoglycemic agents
- smoking cessation
- home visits
- outreach sites
- exercise programs
- foot screening/assessment
Satellite locations:
- registered nurse and one registered dietitian in Cobourg at the
Cobourg Community Centre, 750 Darcy St every Mon, Tue from 9am-5pm
- in Colborne every Wed from 8:30am-4:30pm at 34 Victoria St, Colborne, ON
- Alderville First Nations, twice a month at 8913 Hwy 45, Roseneath, ON
