Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, social worker, case manager, pharmacist, and access to allied health professionals through the family health team

- mental health promotion - group support, psychotherapy, resource counselling, advocacy, and system navigation
- Stop For FHT (family health teams) - smoking cessation program
- Heart Healthy group program
