Service Description: Assist families to enhance their child or youth participation in their community and support the family to advocate for services that they require * Work in partnership with agencies, organizations and families to foster an inclusive continuum of services and facilitate the community's capacity to respond to the family's needs.

Co-ordination Services - Facilitates service planning for a child or youth up to 18 years of age, when the complexity of the child's needs are beyond the capacity of the existing system. Works in collaboration with families and community service providers. Please call, 905 427 7207

Family Networking - Hosts several family events throughout the year such as the annual family day held in September, the children's Christmas party in December and support groups held on an on-going basis.

Enhanced Staffing - Provides additional staffing in licensed child care centres and after school programs to facilitate the inclusion of children with high needs.

Consultation Program - Provides consultation to families in respect to, licensed child care programs, educational setting developmental programming, case management, identification and linkage to other services. This service is available for children 2 - 12 years of age.

Resource Library - An extensive library of books, articles, periodicals and videos pertaining to special needs and is available to both clients and the community.

Dual Diagnosis Respite Funding - For children between 8 to 18 years of age with a mental health and developmental disability.

Transition Activation Worker - Funds to hire a worker to support transition back to community from hopital or residential treatment.
